Job Summary
Senior Social Worker provides psychosocial assessment, discharge planning, case management, and specialized counseling across the care continuum for patients and families in the UW Medical Center-Northwest Social Work Department. Collaborates with patients, families, and interdisciplinary care teams to address psychosocial and emotional needs impacting medical outcomes, facilitates safe transitions between care settings, and supports resource access and advocacy. Requires an MSW (CSWE-accredited) and eligibility for Washington state social work credentials within 90 days; per diem, rotating shifts including weekends/holidays, and variable hours (8-, 10-, 12-hour shifts) with focus on collaboration, communication, coordination, and professional accountability.
Required Qualifications
- Master of Social Work degree from a CSWE-accredited program
- one year of full-time social work experience in a health care setting or equivalent (including practicum placements)
- Within 90 days must hold one of the following credentials issued by the state of Washington: Agency Affiliated Counselor; Licensed Social Work Associate - Advanced; Licensed Social Work Associate - Independent Clinical; Licensed Advanced Social Worker; Licensed Independent Clinical Social Worker
