Job Summary
Assess, care plan, and identify support packages for children and young people aged 0–18 with substantial and permanent disabilities. Collaborate with families, colleagues, and partner agencies to ensure these children have equal opportunities to their non-disabled peers. Work within a multi-disciplinary team of Social Workers, Family Practitioners, and Occupational Therapists to support families throughout their journey with Children's Services. This case-holding role focuses on achieving positive outcomes for children with disabilities within the City and South team.
Required Qualifications
- Is a qualified Social Worker registered with Social Work England.
- Has experience of working with children and families.
- Has strong assessment and relationship-building skills.
- Is committed to achieving positive outcomes for children and young people with disabilities.
