Job Summary
Review juvenile court dockets to identify youth eligible for Proposition 47 diversion services and conduct screenings, assessments, and interviews with clients and families. Coordinate referrals to behavioral health, substance use treatment, housing, and employment resources while facilitating warm handoffs between the court and community providers. Maintain accurate case records, referral logs, and outcome tracking to meet BSCC grant requirements and support quarterly evaluation activities. Apply trauma-informed, culturally responsive practices in all client interactions and ensure compliance with confidentiality regulations and project objectives.
Required Qualifications
- Bachelor's degree in Social Work, Sociology, Psychology, Human Services, Criminal Justice, or a related field
- Three years of relevant experience working with justice-involved youth, behavioral health, diversion programs, or case management
- Minimum of three years of professional experience providing case management, social services, behavioral health, diversion services, juvenile justice services, or related human services
Desired Qualifications
- Experience collaborating with courts, probation, behavioral health systems, schools, and community-based organizations
- Experience with grant-funded programs and outcome reporting
