Job Summary
Assist pharmacists, physicians, and nurses with medication reconciliation processes to ensure differences between pre-admission and admission medications are intentional and free of negative patient incidents. Coordinate discharge reconciliation with pharmacists and case managers while offering discharge prescription services through ProHealth Retail Pharmacy. Collect insurance and demographic data for prescription processing across rotating 10-hour shifts at three hospital facilities. Maintain a valid Wisconsin driver's license and BLS certification to support these duties within a culture focused on exceptional patient care and community service.
Required Qualifications
- High School Diploma or equivalent
- Pharmacy Technician (CPhT) within 2 years of hire and registered with State of Wisconsin
- Valid WI driver's license and in good standing
- Basic Life Support Certification (BLS)
- Weekend and holiday rotation
- Rotating 10 hour shifts across three hospital facilities with varying shift start times of 6 am, 7 am, 930 am, and 1030 am
- 1.0 FTE
- 40 hours per week
