Job Summary
Conduct comprehensive assessments, deliver targeted interventions, and provide treatment for individuals with intricate mental health needs while fostering strong therapeutic alliances with consumers and their families. Collaborate with team members to deliver high-quality clinical treatment, recovery, and trauma-informed support, actively engaging in primary, secondary, and tertiary consultation services alongside community development and education. Maintain thorough case management for people with serious mental illness and complex needs, aligning practices with local policies and strategic directions. This role operates within Hawthorn Continuing Care Team at St Vincent's Hospital Melbourne, utilizing the Strengths Model of Recovery-Oriented Practice to promote consumer well-being.
Required Qualifications
- A minimum of at least 5 years clinical experience within Mental Health Services post qualification
- Experience in the provision of assessment, discipline specific intervention and case management services for people with serious mental illness and complex needs and their carers
- Extensive experience in the delivery of clinical mental health services and preferably in a diverse range of mental health service settings
- Knowledge of relevant legislation, mental health policy directions and principles and practice of community based service delivery including case management
- A valid Nationally Coordinated Criminal History check, or willing to obtain
- You may therefore be required to undergo mandatory immunisations/vaccinations (including flu vaccinations)
