Job Summary
Senior Manager, RN, Care Transitions leads the hospital-based Care Transitions program, serving as the on-site clinical leader and primary Avail Health representative within the hospital. Responsible for day-to-day clinical leadership and people management of the interdisciplinary team, hospital-side patient identification/assessment/enrollment, and coordination with hospital care-management teams to ensure effective transition planning and enrollment, with regular daily virtual huddles for the remote team. Oversees risk assessment and mobilization of interdisciplinary resources, partners with executive leadership on performance, with occasional mobile in-person visits post-discharge. Primarily in-person with limited remote for administrative/leadership activities; reports to Senior Operations Manager and executive leadership; founding leadership role to stand up the program.
Required Qualifications
- Active, unrestricted Maryland RN license; current BLS certification
- 5+ years of clinical RN experience including mobile care delivery and post-hospital transitions, discharge planning, or complex care coordination for high-risk populations
- 2+ years of experience supervising, coordinating, or leading clinical staff or interdisciplinary care teams
