Job Summary
Provide home health PT visits in Senior Living Facilities across Montgomery County, MD; perform mobility and balance assessments, wound care and integumentary interventions, develop and revise individualized care plans including manual therapy, gait training, and safety measures; educate clients and family members on rehabilitative care to promote health, safety, and independent living; document observations and evaluations in a state-of-the-art EMR tablet; must hold MD PT license and graduate from an APTA/CAAHEA-approved program; role supports a multidisciplinary team and field/home-based care.
Required Qualifications
- A current MD Physical Therapist - PT license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
