Job Summary
Conduct home visits to clients in designated territories to perform thorough evaluations for speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on physician orders and multidisciplinary care goals. Educate patients and family members on rehabilitative care to promote independent living. Accurately document observations, interventions, and evaluations using a touch pad tablet. Requires a current New Hampshire Speech Language Pathologist license and solid computer skills with EMR experience preferred. Required Qualifications
- A current license as a Speech Language Pathologist in New Hampshire
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
