Job Summary
Conduct outpatient home health visits to adult and geriatric clients in Franklin and Chittenden County, Vermont. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop and revise treatment plans based on physician orders and multidisciplinary care goals. Educate patients and family members on rehabilitative care to promote independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current Vermont Speech Language Pathologist license and solid computer skills; prior home care experience is a plus. Base pay ranges from $45.00 to $50.00 per hour with flexible hours, no nights or weekends, and a comprehensive benefits package including paid time off, health, dental, and vision.
Required Qualifications
- A current license as a Speech Language Pathologist in Vermont
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
