Job Summary
Follow a designated care plan, make home visits to clients in designated geographic territories, and perform assigned duties including medication administration, wound care, and treatments. Monitor clients' conditions, report changes to Clinical or Client Services Manager, and follow up with, execute, and properly document doctors' orders. Perform client assessments as necessary, manage case coordination, and accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current Vermont RN license with at least one year of recent clinical experience and ability to travel to cases as assigned.
Required Qualifications
- A current license as a Registered Nurse in Vermont
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
