Job Summary
Conduct outpatient visits to clients in designated geographic territories across Orange County, Vermont. Perform diagnostic tests and measurements including mobility, range of joints, transfer status, balance, and safety assessments. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing on multidisciplinary care strategies. Educate clients and family members on rehabilitative care to promote independent living. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current Vermont Physical Therapist license and one year of recent clinical experience. Base pay ranges from $45-50 per hour.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- Current Vermont Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills
