Job Summary
Conduct outpatient home health visits to adult and geriatric clients within designated geographic territories in the Salem, Bedford, and Nashua triangle. Perform diagnostic tests measuring mobility, range of motion, balance, strength, and ambulation patterns to assess safety. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing staff on multidisciplinary care strategies. Educate clients and family members on rehabilitative care and activities promoting independent living. Document observations, interventions, and evaluations accurately using a touch pad tablet. Maintain a current New Hampshire Physical Therapist license with at least one year of recent clinical experience.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current New Hampshire Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills

- Solid computer skills
- Prior experience with electronic medical records (EMR)
