Job Summary
Conduct outpatient visits to clients in designated geographic territories, performing diagnostic tests for mobility, strength, balance, and safety. Develop and implement individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing on multidisciplinary care management. Educate clients and family members on rehabilitative activities to promote independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current New Hampshire Physical Therapist license, one year of recent clinical experience, and graduation from an accredited PT program.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current New Hampshire Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Strong organizational skills
- Strong problem solving skills
- Solid computer skills
Desired Qualifications
- Prior experience with electronic medical records (EMR)
