Job Summary
Conduct outpatient therapy evaluations to develop care plans for adults with intellectual disabilities or in senior living communities. Operate at the top of licensure using research-based approaches to maximize functional independence for clients. Collaborate with home health therapy partners to transition care from home health to outpatient services, ensuring client-centered goals are met in their living environments. Educate clients, families, and community caregivers on care plans to foster a team approach. Perform evaluations, treatments, and discharge planning while documenting individualized treatment approaches using the outpatient EMR platform. Requires a current Vermont Occupational Therapist license, one year of recent OT experience, and proficiency with electronic medical records. Full-time role with base pay between $45.00 and $51.00 per hour.
Required Qualifications
- A current Vermont Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- A minimum of one year of recent work experience as an Occupational Therapist
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
