Senior Integrated Clinic Pharmacist

Clinical Pharmacist with 5+ years of experience in ambulatory/primary care, responsible for evaluating pharmacotherapy for high-risk patients, implementing evidence-based protocols, optimizing drug coverage and costs, collaborating with physicians and nurses, participating in high-risk rounds, providing patient counseling and education, documenting interventions in EMR, supporting CMS Stars and HEDIS measures, conducting staff training, and staying current with pharmacotherapy guidelines. Will support quality improvement initiatives, post-discharge reconciliation, and respond to requests within defined practice scope while contributing to organizational goals. Travel to Humana/CenterWell offices for training or meetings may be required. Provides consultation on complex medication management issues and aids in improving efficiencies and outcomes. Requires a Missouri license with willingness to obtain additional state licenses as needed; advanced degree and related ambulatory/primary care experience preferred; multilingual abilities are a plus.
