Senior Director, Clinical Care Coordination

Job Summary
Lead and manage specialized care and clinical case management teams, ensuring alignment with strategic objectives and driving improvements in care path adherence and client outcomes. Oversee client placement processes, manage referral pathways, and collaborate with provider partners to optimize capacity and ensure timely, equitable service delivery. Provide clinical oversight for high-risk cases and escalations, establish protocols for multidisciplinary collaboration, and monitor metrics to drive process efficiency and accountability. Represent the organization in external engagements, regulatory communications, and stakeholder meetings while partnering with executive leadership to align clinical operations with organizational goals.
Required Qualifications
- 8+ years of experience working in a home-health setting with supervisory responsibilities
- Licensed Clinical Psychologist, LMFT, LPCC or LCSW
- 5+ years of progressive leadership experience in clinical operations, including oversight of clinical operations, access and client care management
- Experience working with individuals with autism spectrum disorders (ASD) and related developmental disabilities
- Proven success in strategic planning, program development and managing remote teams
- Experience interpreting and implementing policies, procedures, and regulations of CCS
- Highly Proficient in Microsoft Office Suite (Outlook, Word, Excel, etc.)
- Strong analytical skills
- Strong interpersonal skills
- Ability to obtain and maintain criminal record clearance through the Department of Justice (DOJ)
- Ability to obtain and maintain clearance through the Office of Inspector General
- Must attend any required training
- Must be able to communicate effectively within the work environment
- Visual and auditory ability to work with clients, staff, and others in the workplace continuously
- Frequent speaking and listening (25-75%) to clients, staff, and others in the workplace
- Ability to utilize computer, cell phone (iPhone)
- Frequent sitting
- Ability to utilize virtual meetings software (Zoom, Teams, etc.)
- Knowledgeable in evidence-based methodologies found to benefit individuals diagnosed with developmental disabilities, including Applied Behavior Analysis (ABA), Early Start Denver Model (ESDM), Treatment and Education of Autistic and related Communication-Handicapped Children (TEACCH), Picture Exchange Communication System (PECS), and Pivotal Response Training (PRT)
- Training in the field of ABA/Behavioral Health Treatment
- knowledge of other community resources and agencies that serve children
- Knowledge of multiple historically excluded groups with knowledge in I/DD (intellectual and developmental disability) communities
- Experience working and communicating with families
- Sensitivity to working with an ethnically, linguistically, culturally, and economically diverse population
- Commitment to meet agreed upon deadlines on time or ahead of schedule
