Job Summary
Senior Clinical Care Navigator, Cardiology leads complex cardiology patient journeys and cross-functional coordination across clinical and operational teams. You will manage a dedicated caseload, facilitate referrals for testing and procedures, develop and evaluate educational materials and care paths to ensure adherence to medical plans, and own the end-to-end success of the service line. The role involves creating scalable SOPs to empower the team, acting as a Change Champion as processes and tools evolve, serving as a liaison between COE clinical experts and operations, and collaborating to optimize workflows for improved patient experience and outcomes. Requires BSN with RN licensure (8+ years in cardiology), strong data/reporting skills (Excel/PowerPoint), adaptability to a fast-paced startup environment, HIPAA compliance, and the ability to learn new software. Remote-first with US-wide presence and offices in San Francisco and Chicago.
Required Qualifications
- Registered Nurse (RN) with BSN
- 8+ years of progressive clinical experience in cardiology
- Current and unrestricted RN licensure
- Ability to collect, report, and present program metrics (Excel, PowerPoint)
- Experience leading interdisciplinary teams and managing specialty clinics
- HIPAA compliance and confidential information handling
- Ability to learn new software and work across tech platforms
