Job Summary

Senior Case Manager III leads and mentors the case management team to arrange, coordinate, monitor, and assure services for clients. Responsibilities include: conducting comprehensive screenings and assessments of client/household needs across functional, environmental, financial, employment, housing, educational, and health domains; developing and coordinating service plans; scheduling appointments; providing necessary follow-up; conducting home and field visits as needed; monitoring quality of services; auditing case files for completeness and accuracy; crisis intervention; coordinating referrals to VA/community providers for therapeutic, social, educational, and medical resources; providing counseling and advocacy; ensuring documentation and secure records; developing and delivering workshops on program services; participating in outreach events; mandatory on-site work; and والن compliance with mandated reporting requirements. Education and experience requirements include a Bachelor's degree in the social sciences or family therapy, five years of case management experience (master’s degree in the social sciences reduces to one year), and preferred Master’s in Social Work or related field with supervisory experience. The role is with Volunteers of America Los Angeles and requires on-site presence and adherence to E-Verify, with a focus on serving veterans and high-need populations.

Required Qualifications

  • Bachelor’s degree from an accredited institution in the social sciences or family therapy
  • Completion of VOALA Case Management training within 6 months of hire
  • Minimum total of five (5) years of demonstrated case management experience with high needs/high intensity of service populations
  • If Case Managers possess a master’s degree in the social sciences, only one (1) year of case management experience is required
  • Must be able to pass a fingerprint clearance, background check including criminal history, personal references, employment and education verifications
  • Current TB screening and Immunization Records (within 5 days of hire)
  • Copy of Current Professional License (if applicable)
  • Current BLS Certification (within 15 days of hire)
  • Valid Real ID or US Passport
  • Experience with HMIS/CM process flow
  • Completion of required trainings (within 30 days of hire)
  • Ability to perform under emotional stress and conflicts involving high-needs clients
