Job Summary
Conduct home-based case management and supportive mental health services for formerly chronically homeless families with special needs throughout Contra Costa County. Perform comprehensive psychosocial, life skills, and financial assessments to develop and monitor individual service plans, while providing linkage to medical, mental health, addiction, legal, and community resources. Maintain regular contact with consumers via home visits and office-based coordination, assisting with budgeting, benefit access, and housing stability negotiations. Manage data entry in HMIS/Apricot, generate reports, and submit subsidy requests, acting as the point of contact for contract liaisons and external meetings. Collaborate with staff on onboarding, training, and shadowing opportunities, ensuring compliance with HIPAA, housing justice principles, and all federal and state regulations. On-call availability and after-hours support may be required.
Required Qualifications

- SHRM-SCP certified professional
- Valid driver's license
- Ability to travel
- Ability to work on-site for the first 90 calendar days
- Ability to work on-call, after-hours, and backup
- Ability to structure workday with approximately six hours of face-face work and approximately two hours of administration/documentation time

- Ability to provide linkage for medical, mental health, addiction/recovery, financial, legal, and other community services
- Ability to negotiate housing issues with existing landlords/property managers
- Ability to collaborate closely with all providers involved in supporting consumers
- Ability to assist consumers to locate units with landlords/property managers willing to accept voucher payments
- Ability to comply with all legal/ethical professional guidelines for maintaining consumer confidentiality, adhering to HIPAA guidelines
- Ability to comply with all local, state, and federal regulations and Hope Solutions policies and procedures
- Ability to complete required administrative paperwork and reports in a timely and accurate manner
- Ability to attend all required meetings, including but not limited to Hope Solutions staff meetings, team meetings, consumer case conferences, and linkage meetings with other agencies
- Ability to document all services provided in a timely manner
- Ability to assist with data generation for reports and funding applications
- Ability to professionally represent Hope Solutions in all circumstances
