Job Summary
Manage temporary and complex cases, ensuring continuity of care during staffing transitions by stabilizing cases, correcting work, and providing coverage for turnover. Deliver person-centered case management services to individuals with disabilities, mental health conditions, or terminal illness across multiple counties, assisting with new County Transfers and setting up support plans. Advocate for persons supported to achieve goals through community resources, rehabilitation programs, and waiver services while monitoring Medicaid eligibility and managing allocations within budget. Lead the team by offering shadow visits to new hires, prioritizing unfinished tasks during program transitions, and discharging or reassigning cases once stabilized. Document all services and billing within one business day in the electronic health record, responding to all communication within one business day. Travel throughout the Twin Cities Metropolitan area, meeting face-to-face at least twice annually to assess needs and adapt support plans.
Required Qualifications
- Bachelor's Degree in social work, nursing, psychology, sociology or a closely related field from an accredited four-year college or university
- A bachelor's degree from an accredited four-year college or university in a field other than social work, nursing, psychology, sociology or a closely related field AND one year of experience in the delivery of social services to the target population as a supervised social worker in a public or private social services agency
- If you hold a Social Work degree, be licensed by the Minnesota Board of Social Work
- Proven record of consistently exceeding minimum billing standards
- Demonstrate strong written communication skills
- Complete documentation of all services provided in the selected electronic health record/database system within one business day of providing services
- As a mandated reporter, respond and report all incidents that occur while providing services to protect the health and safety of and minimize risk of harm to the person(s) supported following Accord's policy and MN Statutes
- Meet completion deadlines for all required and assigned training related to the position
- Comply with all Accord policies and procedures, rules and regulations of the State of Minnesota, and requirements of any contracted or funding parties
- Travel throughout the Twin Cities Metropolitan area to job sites and meeting locations; limited overnight travel possible
