Job Summary
Senior Housing Case Manager duties include case management and supportive care coordination for approximately 30 asylee families, performing psychosocial assessments, developing treatment plans, assisting with obtaining/retrieving vital documents, applying for benefits and entitlements, and housing placement. Responsibilities also cover maintaining client records, providing referrals and follow-ups across primary care, substance use, nutrition, dental, mental health, and financial management, coordinating with internal departments (ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Legal, Job-Training), assisting clients in moving into apartments, supporting transitions from homelessness to stable housing, developing strength-based individualized goals with clients, monitoring progress via regular check-ins and ILPs, and completing required reports. The role requires in-office work 100% of the time, travel within the NYC area, and bi-lingual Spanish skills are strongly preferred. Proficiency in Microsoft Office (Word, Outlook, PowerPoint, Excel) and CARES system is expected, along with strong confidentiality, problem solving, and relationship-building abilities. Required Qualifications
- Bachelor’s degree in social work, psychology, counseling, or related field, or three years of relevant experience in a social service setting.
