Job Summary
Build and maintain referral relationships across hospital discharge teams, skilled nursing facilities, home health agencies, hospice providers, elder law attorneys, and other aging-focused professionals. Serve as a trusted resource for care teams navigating complex transitions — someone they call because they know you’ll get it right. Identify and pursue new outreach opportunities across the Twin Cities market. Represent Oasis at community events, healthcare settings, and professional networks. Collaborate with our advisors to ensure seamless handoffs and strong family outcomes.
Required Qualifications
- 2+ years in business development, liaison, or community outreach in hospice, home health, senior living, or hospital/SNF setting
- Existing relationships across the Twin Cities healthcare landscape
- Deep understanding of the continuum of care: from acute and post-acute through long-term placement and end-of-life support
- A natural relationship-builder who earns trust quickly, follows through consistently, and communicates with clarity and warmth
- Self-directed and comfortable managing your own schedule, pipeline, and territory
- Must be located in the greater Minneapolis/St. Paul area
