Job Summary
Provide Heal at Home services to geriatric patients following an acute inpatient stay. The Advanced Practitioner (Nurse Practitioner or Physician Assistant) conducts medical evaluations, treatment, counseling, referrals, and participates in patient care coordination under supervision and/or collaboration with the attending physician. Responsibilities include diagnostic evaluations, therapeutic remediation, surgical assistance, pharmaceutical prescribing, patient advocacy, participation in quality/performance improvement, virtual care visits as needed, and education of patients, families, and students. Requires maintaining licensure/certification and ongoing professional development; proficiency with electronic medical records and teamwork with multidisciplinary staff. Location: Center Valley, PA (on-site).
Required Qualifications
- Current license to practice in the state of Pennsylvania or New Jersey (as determined by job location) for Certified Registered Nurse Practitioner or Medical Physician Assistant
- NCCPA certification for Physician Assistants
