Senior Care Coordinator, Therapeutic Services responsible for delivering quality, patient-centered mental health/therapeutic services to a caseload of up to 30 community members; works under supervision of the Assistant Care Manager, Therapeutic Services within the Supportive Services team; role supports data collection and research activities, adherence to fidelity monitoring protocols, development of engagement resources, drop-in mental health services, and dissemination of research through reports/manuscripts; requires collaboration across teams, proficiency with Microsoft applications, and ability to manage multiple projects with confidentiality.
