Job Summary
Senior - CARE COORDINATION II at Innovation Care Partners leads and mentors Care Coordinators, coordinates daily operations within the care management team, and oversees onboarding/training, readmission risk assessment, and outpatient care processes. Responsible for scheduling and delivering educational classes, ensuring effective post-hospital telephonic monitoring, supervising office visits, maintaining community resource information, conducting readmission analyses, and coordinating transitions of care with PCPs, health coaches, and care managers. Requires Bachelor-level (or higher) education, BLS certification, Arizona Fingerprint Clearance Card, 3+ years as a Care Coordinator in IOCP contexts (4+ years in outpatient settings), and knowledge of Medicare/Medicaid benefits and regulatory requirements. The role emphasizes mentoring, cross-team communication, and maintaining compliant, integrated care services across aLarge healthcare system in the Phoenix area.
Required Qualifications
- Education: Associates or health care related advanced education
- Required Bachelors or other advanced certification
- Basic Life Support (BLS) Certification
- Fingerprint Clearance Card (FPC) - Current and valid State of Arizona
- Experience: 3 years as Care Coordinator or related role using IOCP model; 4 years as a Care Coordinator in outpatient clinical setting
- Knowledge of Medicare/Medicaid/SSI/SSDI benefits and compliant care transitions
- Ability to mentor/train Care Coordinators and manage a caseload
