Job Summary
Conduct field visits to Medicaid and Medicare members in homes or long-term care facilities while monitoring case management plans against personal goals. Precept, coach, and mentor team members through training initiatives, and provide daily reports on outstanding cases and compliance dashboards. Support audit initiatives, review field work visits, and coordinate services by facilitating interactions with providers and interdisciplinary teams. Ensure patient progress through continuous assessment, data review, and active care planning, guiding families toward appropriate resources.
Required Qualifications
- Bachelor's degree in social sciences, social work, human services, or a related field
- Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one (1) year experience in conducting comprehensive assessments and provision of formal service for the elderly
- Licensed Professional Counselor (LPC)
- Licensed Social Worker (LSW)
- Must reside in the State of Illinois
- 2+ years of experience in health care and/or case management
- 2+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations
- Ability to use a variety of electronic information applications/software programs including electronic medical records
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
- Screening for TB
Desired Qualifications
- Master's degree in social sciences, social work, human services, or a related field
- CCM certification
- Experience with health promotion, coaching, and wellness
- Expertise of community health and social service agencies and additional community resources
