SEEDS Family Support Coordinator

Job Summary
Gather referral and assessment information for the SEEDS Team while monitoring family progress and connecting youth to educational and community support systems. Coordinate existing and added services to maintain fidelity to the SEEDS model, evaluate Family Support Plan effectiveness, and ensure quality documentation of all activities. Manage a caseload of 20-25 families, adhere to virtual office procedures, and participate in the SEEDS Team and continuous quality improvement efforts. Requires a Bachelor's degree in Social Work or related field with one year of children's services experience, a valid Florida driver's license, and successful completion of FDLE clearance and annual training on topics including child abuse reporting and diversity.
Required Qualifications
- Bachelor's Degree in Social Work, Human Services or related field from an accredited university or college preferred with at least 1 year of experience in children's services; or a High School diploma plus three (3) years' experience in children's services or education
- Must possess and maintain a valid Florida driver's license with no record of criminal driving offense of license suspension
- Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHN's current auto insurance policy
- Must be able to work flexible hours, including evenings and weekends
- Must successfully complete and maintain FDLE clearance, Federal background and state criminal background checks, and sexual predator screening
