Job Summary
RN Case Manager I provides comprehensive home-based case management for SCO members with complex needs, developing, implementing, and evaluating individualized care plans to ensure cost-effective quality of care. Responsibilities include assessing patient needs, coordinating with health team, maintaining caseload, overseeing plan implementation, communicating with care staff and providers, educating patients and families, participating in knowledge sharing, and performing other duties as assigned. Requires MA RN license, BSN preferred, 3+ years of RN experience (home care preferred), proficiency with Microsoft Office, experience with case management data systems, and ability to travel locally. Occasional travel to support home-based care and collaboration with care teams. Benefits include health, dental, vision, PTO, retirement, and supplemental benefits.
Required Qualifications
- Registered Nurse currently licensed in the Commonwealth of MA
- Bachelor of Science degree in Nursing preferred
- Minimum of three (3) years experience as a licensed, registered nurse, preferable five (5) years, including home care clinical experience
- Proficient in the use of Microsoft Office tools
- Experience working with case management data systems
- Ability to travel locally on a frequent basis
- Covid vaccinated preferred
- Spanish Speaking preferred
