SCO Nurse Care Manager - Bilingual preferred - Springwell Metrowest (Framingham, Marlborough, South/North/Westborough, Hudson, Sudbury, Wayland, Ashland, Natick)

RN Care Manager (Nurse Care Manager) ensures timely care management across the continuum, including transitions of care, care coordination and navigation, complex case management, population health and disease management. Conducts telephonic and face-to-face outreach, develops member-specific plans of care, documents per policy, provides targeted health education, facilitates safe transitions in care, and collaborates with caregivers, providers, and the interdisciplinary team to improve outcomes; participates in interdisciplinary rounds and demonstrates strong clinical and communication skills.
