SCN Screener and Navigator

Contact clients to confirm demographic and household information, obtain consent, and verify Medicaid eligibility. Conduct Social Care Network screenings and eligibility assessments, then identify eligible services to assist with referrals. Create and manage cases within the SCN platform, follow up on outreach, and document all activity while maintaining accurate, compliant records. Coordinate with providers and team members to ensure clients successfully receive services. This role supports LaSante Health Center's Medicaid members through the Social Care Network process, requiring strong communication skills and experience with care-management platforms like Unite Us.
