Job Summary
School to Work Counselor responsible for coordinating and delivering transition services, facilitating intakes and individualized service plans, delivering life skills instruction and job training, and connecting students with community-based work experiences. Requires collaboration with families, school personnel, and agency partners; strong documentation and data-tracking skills; proficiency in Microsoft Office; valid driver's license and Real ID; and a Bachelor's degree in a related field with 2–3 years of experience supporting individuals with disabilities. The role is primarily on-site in New Britain, CT, with extended responsibilities including travel to schools and community sites to support student independence, employment readiness, and successful transitions to adulthood.
Required Qualifications
- Bachelor's degree in education, human services, rehabilitation, or a related field
- Minimum of 2 to 3 years of experience supporting individuals with disabilities—ideally youth or young adults transitioning to adult services
- Knowledge of vocational training, job coaching, and employment support services, along with basic data tracking and reporting skills
- Proficiency in Microsoft Office (Outlook, Word, Excel, Teams) and a valid driver's license
- Valid driver's license
- Valid Real ID
