Job Summary
Assess, plan, implement, and evaluate educational programming for students enrolled in the Diabetes and School Health program while traveling to schools in Columbus and surrounding areas. Conduct chronic disease follow-up visits according to care algorithms, manage medication regimes, obtain prior authorizations, and transport medications. Facilitate skill development activities, mentor school nurses, and present evidence-based research to staff. Collaborate with families, primary care providers, and specialty teams to coordinate discharge planning and psycho-social support. Maintain clinical competency, participate in program recruitment, and drive data collection to improve patient care. Requires a Bachelor's in Nursing, Ohio RN licensure, and a valid driver's license. Schedule is 32 hours per week, M-F day shift.
Required Qualifications
- Valid Ohio driver's license
- Proof of auto insurance
- Pass motor vehicle background inspection
- Insurance eligibility
- Driving qualifications
- Training set forth by Nationwide Children's Hospital
- Maintain qualification of insurance guidelines
- Bachelor of Science in Nursing
- Registered Nurse with current licensure to practice nursing in the State of Ohio
- Basic-Life Support (BLS) certificate
- Demonstrated nursing competence with specific clinical skills and knowledge in disease management
- Advanced level of critical thinking
- Organizational skills
- Decision-making skills
- Excellent verbal and written communication skills
- Computer skills to maintain program and patient data
- 4 years of nursing experience
