School Based Therapist, LSW or LPC

Therapist position in a school-based program within NYAP's STAR team at Heath City Schools (Heath, OH). Responsibilities include providing individual, group, and family mental health services, case management, and transportation as needed; traveling to deliver community-based services for youth and families; ensuring timely documentation and achieving productivity thresholds; coordinating and monitoring services across mental health, medical, educational, psychological, vocational, and social domains; regularly reviewing and updating youth plans; fostering strong relationships with foster parents and families; supporting family reunification or stable living arrangements; delivering safety assessments, crisis response, and behavioral stabilization services; participating in Continuous Quality Improvement activities and weekly supervision. Minimum qualifications include a Master’s Degree in Social Work or related field, two years’ experience with youth and families (preferred), a valid advanced professional license as required, and a valid driver’s license with appropriate insurance and driving record.
