Job Summary
School-Based Support Specialist delivers intensive outpatient case management to youth in a school setting, coordinating care with schools, families, and providers. Provides direct, individualized interventions aligned with treatment plans to promote recovery, resilience, and academic achievement. Services are primarily delivered on school premises during school hours, with additional support in homes and the community as needed. Responsibilities include crisis intervention, outreach for missed appointments, documenting services timely and accurately, and collaborating with internal and external partners to ensure client-centered care. Requires knowledge of case management, rehabilitation methods, treatment planning, and electronic health records; strong communication, time-management, and ability to work independently within agency standards.
Required Qualifications
- Bachelor's degree in psychology, social work, behavioral health, or related human services field
- Bachelor’s degree in an unrelated field with two years of related behavioral health experience
- Any four-year combination of higher education and two years of qualifying experience
- Associate’s degree in applied science in behavioral health support
- Four years of full-time qualifying experience in the field
