School Based Services Client Support Provider II

Job Summary
Provide services in community settings including schools, homes, and shelters by delivering outreach, support, referrals, and individual therapeutic interventions. Develop and document person-centered treatment plans with clients and families while monitoring progress and adjusting activities as needed. Advocate for client rights, preserve confidentiality, and manage crisis situations or coordinate emergency services. Conduct strengths assessments, implement scheduled day activities, and provide education on mental illness upon request. Maintain clinical schedules in the electronic health record, submit incident reports, and attend clinical supervision and program staffing. Drive to community locations to transport clients and comply with all agency policies, ethical codes, and state training requirements.
Required Qualifications
- Must be at least 21 years old
- Bachelor's degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education
- Strong oral and written communication skills using the English language
- A valid driver's license and automobile liability insurance maintained in accordance with Pawnee policy and procedure
- Basic personal computer skills which enable the entry, retrieval and use of electronic data
- All employees will be required to submit to and pass a background check and drug screening
Desired Qualifications
- Experience with Microsoft Office software preferred
- Some collegiate-level schooling in a human services field
