School-Based Community Health Worker

Serve as Care Alliance Health Center's primary Community Health Worker within assigned schools, building trusting relationships with students, families, and school staff. Promote awareness of medical, dental, behavioral health, and vision services while assisting eligible families with enrollment and accessing specialty care. Conduct Social Determinants of Health screenings, coordinate referrals and follow-up care, and connect individuals with resources for transportation, food insecurity, and housing. Deliver age-appropriate health education on nutrition, physical activity, and preventive care. Document all outreach, screenings, and case management activities in electronic health records. Occasional evening or weekend participation in school events may be required, with regular travel between schools and community partners.
