Oversee care coordination for a caseload by creating shifts that maximize coverage based on caregiver skills and client preferences, while monitoring schedules for cancellations and rescheduling needs. Serve as the primary contact for patients and families regarding care coordination, communicate pertinent patient information, and maintain accurate electronic and physical records. Complete required documentation for complaints or incidents, ensure compliance with privacy regulations, and participate in care conferences to update patient status.
