Job Summary
Dedicated Full-Time Home Health Scheduler to coordinate patient/client services in the Albuquerque, NM metro area. Responsibilities include scheduling and documenting patient/client schedules, coordinating care with management and interdisciplinary teams, communicating with patients, families, and healthcare professionals, and ensuring timely reports and compliance. Requires strong communication, multitasking, organization, confidentiality, and experience with electronic medical records (HCHB preferred). Office-based role with flexible scheduling and extended hours; occasional lifting up to 50 pounds is possible. Required Qualifications
- High School graduate
- One year's experience as a home health or hospice scheduler highly preferred.
