Job Summary
In-home family support role serving families in Waukesha County, WI, with duties including delivering education, resources, and biopsychosocial assessments to support child wellbeing and family health. Responsibilities cover conducting home visits, developing family-centered goals, coordinating referrals and warm handoffs, providing psychoeducation and coaching using teach-back methods, promoting independence and protective factors, collaborating with community partners, ensuring safety and confidentiality per federal/state requirements, and maintaining documentation. Requires a Bachelor’s degree in a human service field, travel between sites, and a home-based hybrid schedule with potential evening/weekend hours; bilingual English/Spanish is a plus. Strong emphasis on trauma-informed care, effective communication, and culturally humble practice; familiarity with Healthy Families America and interagency collaboration is expected.
Required Qualifications
- Bachelor’s degree in a human service field
- Experience working with or providing services to children and families
- Willingness to travel between multiple sites
- Available to work flexible hours in a home-based hybrid model
- Possess a valid driver’s license and reliable transportation
- Criminal background check
- Bilingual in English/Spanish is a plus
