SBHC Nurse Practitioner/Physician Assistant (.80FTE)

Job Summary

Provide medical triage, care management, and patient education to a diverse panel while delivering evidence-based care for acute, preventive, and chronic conditions. Make referrals to specialty providers, order diagnostic tests, and conduct in-home assessments as needed. Serve as a cross-functional team member managing complex cases, chart reviews, and case conferences alongside physicians and interdisciplinary staff. Work evening and Saturday rotation clinics with flexibility to meet patient needs. Maintain accurate records and collaborate with medical assistants, nurses, and case managers to improve clinic flow and service delivery.

Required Qualifications

  • Knowledge and experience with Electronic Health Record programs, such as NextGen or Epic
  • High degree of interpersonal competence with both staff and patients
  • Thorough knowledge of evidence based care
  • Cultural competence working in diverse/low income communities
  • Familiarity with Electronic Panel Management
  • Ability to deal courteously and effectively complex patient profiles
  • Must have the physical stamina to function effectively in a position that involves heavy lifting (50lbs)
  • Ability to work courteously and effectively as a team member
  • Ability to work independently
  • Exercise good judgment
  • Communicate effectively orally and in writing
  • Make and thorough manner with speed and accuracy with minimal supervision
  • Ability and willingness to work under pressure

  • Must hold a current BLS (Basic Life Support) Certificate
  • Requires board certification
  • Must become board certified within 6 months of employment
  • Requires valid California license
  • Requires DEA
  • Requires a current license from the California Board of Registered Nursing
  • Requires a national certification from the American Academy of Nurse Practitioners or other national certifying bodies
  • Requires a certificate of completion of a Nurse Practitioner Program
  • Requires a minimum of 2 years clinical experience (Nurse Practitioner)
  • Requires a current license from the California Physician Assistant Committee
  • Requires graduation from an approved program of instruction in primary health care
  • Requires having passed a certification examination administered by the Bureau of Medical Quality Assurance
  • Requires a minimum of two years of full-time clinical experience after clinical practicum (Physician Assistant)
  • Emphasis in the areas of family medicine or geriatrics (Physician Assistant)
