Job Summary
Process referrals from various sources and compile documentation to transition care from acute or sub-acute settings to BAYADA as ordered by a physician. Partner with the Transitional Care Manager, Marketing Manager, and area sales leadership to build relationships with physicians and healthcare professionals while coordinating service setup with the service office. Demonstrate core BAYADA values, participate in weekly sales meetings, and exceed performance standards in key job dimensions. Manage projects in a fast-paced environment, multitask effectively, and communicate clearly in English to support the success of the Home Health Sales team.
Required Qualifications
- Bachelor's degree preferred
- 1 year of experience in a healthcare setting
- home health care preferred
- Experience with any health system / referral management
- home health platforms a plus
- Ability to multitask in a fast paced environment
- Demonstrated ability to manage projects
- Demonstrated record of successfully taking on more responsibility with positive results
- Ability to read, write and effectively communicate in English
- A demonstrated record of strong interpersonal skills and goal achievement
- Ambition to grow and advance beyond current position
- Strong PC and communication skills
