Salaried Physical Therapist Full Time 15K Sign On Bonus

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, documenting client performance, modifying goals, and determining discharge care plans. Collaborate with physicians, nurses, and caretakers to educate patients on home exercise plans and coordinate services across the healthcare team. Supervise personnel including PTAs and HHAs where applicable, and complete OASIS assessments as permitted by state regulations. Maintain effective communication with staff and families while serving as a clinical resource consultant.
