Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals. Provide written evaluations and plans of care defining needs, progress, and expected outcomes. Maintain discharge plans by collaborating with healthcare teams, educating caretakers on home exercise plans, and completing OASIS assessments. Supervise personnel including PTAs and HHAs as applicable. Ensure accurate documentation to prevent technical or clinical denials while adhering to all licensure regulations and patient care standards.
