Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations defining physical therapy needs, progress, and expected results. Deliver services complying with physician orders while maintaining plans of care through timely documentation, goal modification, and discharge planning that includes educating patients and caretakers on home exercise routines. Coordinate services with physicians, nurses, and families, supervise personnel as applicable, and complete OASIS assessments where permitted. Ensure documentation meets payer standards to prevent technical or clinical denials and comply with all licensure regulations and patient care standards.
