Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, modify treatment as needed, and coordinate discharge planning with healthcare teams and caregivers. Document client performance, complete OASIS assessments, and maintain effective communication with staff and families. Supervise personnel including PTAs and HHAs where applicable. Requires a Physical Therapy degree, current unrestricted state license, and valid CPR certification. Full-time home health role with a $15,000 signing bonus.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Drivers License
- Proof of Automobile Insurance
- Required vaccinations
- Testing
