Salaried Physical Therapist Full Time 15K Sign On Bonus

Conduct initial and ongoing patient assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to establish functional goals and outcomes. Provide written evaluations and plans of care, documenting client performance, modifying goals, and determining discharge care plans. Collaborate with the healthcare team, physicians, and caretakers to coordinate services and educate patients on home exercise plans. Supervise personnel such as PTAs and HHAs where applicable and complete OASIS assessments per state regulations. Maintain effective communication with staff and families while adhering to all policies, licensure regulations, and patient care standards.
