Salaried Physical Therapist Full Time 15K Sign On Bonus

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to define goals and outcomes. Provide written evaluations, modify treatment plans, and manage discharge care including patient education on home exercise routines. Complete OASIS assessments, maintain accurate documentation to prevent denials, and collaborate with physicians, nurses, and family members. Supervise staff such as PTAs and HHAs as applicable. Requires a Physical Therapy degree, current state license, and CPR certification. Full-time role with a $15,000 signing bonus.
