Job Summary
Conduct initial and ongoing mobility assessments to evaluate strength, coordination, and functional goals. Deliver physical therapy services complying with physician orders while maintaining accurate OASIS documentation to prevent clinical denials. Develop written plans of care defining needs, progress, and expected outcomes, then modify treatments and determine discharge care plans as appropriate. Collaborate with physicians, nurses, and family members to coordinate services and educate caretakers on home exercise plans. Supervise physical therapy assistants and home health aides where applicable.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Drivers License
- Proof of Automobile Insurance

- Experience in Home Health Therapy
- completion of OASIS documentation
