Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, goals, progress, and expected outcomes, ensuring documentation results in no technical or clinical denials. Maintain discharge care plans by collaborating with physicians, nurses, and caretakers to educate patients on home exercise regimens. Supervise personnel including PTAs and HHAs where applicable, and complete OASIS assessments where permitted by state regulations.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Valid Drivers License
- Proof of Automobile Insurance
- Required vaccinations
- Testing
Desired Qualifications
- Experience in Home Health Therapy
- Completion of OASIS documentation
