Salaried Physical Therapist Full Time 15K Sign On Bonus

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels for home health patients. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Maintain plans of care through timely documentation, modifying treatment as needed, and creating discharge plans with patient and caretaker education. Collaborate with physicians, nurses, and other team members to coordinate services and complete OASIS assessments where permitted. Supervise PTAs and HHAs as applicable, ensuring all documentation meets payer standards to prevent denials. Adhere to Aveanna Healthcare's policies, licensure regulations, and patient care standards while communicating effectively with staff and families.
